SPRING Social Prescribing

SPRING social prescribing is a partnership between the Healthy Living Centre Alliance and Scottish Communities for Health and Wellbeing.

Social prescribing acts a a link between the health service and the community.

The project has a network of 30 community health partners delivering Social Prescribing service. Annexe Communities is one of the partners and serves the Partick area of Glasgow, taking referrals from the local GP cluster groups and other health professionals.

At Annexe Communities our SPRING social prescribing project is a way to link medical care to non-clinical, locally delivered support services. We enable medical professionals to refer their patients to a range of community-led activities and services, such as yoga, cooking groups, art classes and walking groups - recognising a social model of health - where wellbeing is promoted and measured in different ways.

Social prescribing empowers patients, supports greater independence, reduces reliance on primary healthcare and allows individuals to get involved in community-led activities to benefit their health, wellbeing and social interactions.
